Economics of Medical Education: An Important Dimension in India's NEET Reform Debate

After weeks of protests at Jantar Mantar, disruptions during the Monsoon Session, and an intense national debate over examination reforms, Parliament on Tuesday (July 28) finally began discussing the Public Examinations (Prevention of Unfair Means) Amendment Bill, 2026. Unsurprisingly, much of the discussion has centred on strengthening examination security, accountability and preventing paper leaks.

Having spent over a decade covering NEET, medical admissions, NMC, NBEMS and higher education policy as an education journalist, I have followed this issue through multiple policy cycles. During those years, I noticed that discussions around paper leaks almost always focused on security, technology and accountability. Rarely did they extend to the economics of medical education itself.

Across parliamentary discussions, television debates and expert forums, several constructive suggestions continue to emerge—strengthening examination processes, identifying leak points, improving confidentiality, enhancing penalties, increasing accountability and expanding MBBS seats. These are all important reforms and deserve serious attention.

However, examination integrity does not exist in isolation. It is closely connected with the broader ecosystem of medical education.

Medical education today sits at the intersection of healthcare, education, public service and economics. While India has significantly expanded medical education through both public investment and private participation, affordability continues to remain an important policy discussion. When the financial stakes associated with medical admissions become extraordinarily high, it is worth asking whether conversations on examination reforms should also acknowledge the economic realities surrounding medical education.

This is not an argument against private investment. On the contrary, private institutions have played a significant role in expanding India's medical education capacity and strengthening healthcare infrastructure. The next policy challenge is ensuring that quality, accessibility, affordability and sustainability evolve together.

Medical education is unlike most professional courses because it directly shapes the country's future healthcare workforce. Every policy decision in this space ultimately influences patients, hospitals, public health systems and the quality of healthcare delivery.

Perhaps the next phase of NEET reforms is not only about making examinations more secure, but also about making the entire medical education ecosystem more balanced, accessible and future-ready.
